Step 1: Complete claim form

A completed Travel Insurance Claim Form

Step 2: Prepare required documents Please note that the list of documents below is not exhaustive. Other documents may be requested if necessary. For the complete list of required supporting documents, please refer to the claim form.

Death/ Permanent Disability Claim

• A copy of boarding passes and itinerary

• Death certificate, autopsy report, coroner’s findings

• Proof of relationship between the insured and the claimant, e.g. copy of marriage certificate / birth certificate

• Police report, if applicable

• Medical report

Hospital & Surgical Expenses

• A copy of boarding passes and itinerary

• Original inpatient bills and receipts

• Inpatient discharge summary/ medical report

Loss of Personal Effects Claims

• Submit a police report or an incident report of the loss.

• Submit original proof of purchase and/or warranty cards or invoices.

• For Theft / Loss claims submit a list of items stolen with the information on make/model, date of purchase and purchase price • Submit photographs of damaged items if any

• Claim amounts will be adjusted for depreciation or wear and tear during assessment. The amount will be further adjusted if you are not able to provide any supporting documents.

• For loss of mobile phones, submit letter showing that you have terminated the phone line

Damage to Luggage Claims

• Submit a Property Irregularity Report (PIR

• A detailed list of claims for the damaged/lost property

• Police Statement and Report if police is involved

• Request for a PIR at the airport when you discover that your luggage is damaged

• Take photographs of the damaged luggage.

• Submit repair bill if the luggage was repaired.

• You cannot claim under the policy if the airline has already replaced your damaged luggage or repaired your luggage.

Step 3: Submit the completed claim form and required documents.
